REUNION RESERVATION FORM

For RESERVATIONS at Hotel Caribe Royale ? You will need to be a member of
the Panama Canal Society!! To register for the Society and their Annual
Reunion, which includes two dances - on Friday and Saturday evenings with
Panamanian music, visit their website at: www.pancanalsociety.org . (The
hotel is approximately 2 miles from Disney World and the Marriott World
Center Resort.)

WELCOMING RECEPTION: A small informal gathering is being planned for the
evening of Wednesday, July 28, 2004 at the hotel. More information to
follow. DRESS: Casual.

* IMPORTANT NOTES:
· Please mail the reservation sheet with your check to the
address indicated by April 30, 2004. We have to sign a contract with the
hotel & need an approximate head count ASAP. Thank you!

Cancellations with full refunds will be honored prior to June 1, 2004.

Spouses and significant others are invited with registration form and
payment.

Non-classmates and other friends are welcome to join us but need to
register and pay by the deadline

or they can also join us later at 10:00pm for dancing and live music
with a small fee at the door.

No children under the age of 18 will be permitted to attend our class
function. Sorry!
